Would that have meant he wasn't on the hand-written Banzuke?

Kyujo has nothing to do with the banzuke appearance.  An intai would do it.

So a rikishi with a broken leg, say, still automatically appears on the banzuke released at the start of the tournament? I hadn't realised that.

Even retired rikishi sometimes appear on the banzuke. Witness Kotoyuuki.

If the retirement is announced during a basho or a couple days later, the rikishi will not appear on the next banzuke.  Once the shimpan have met to set the next banzuke, it is very rare for a rikishi to be removed.  It usually takes some kind of scandal, and then there is just a blank spot left for the rank the rikishi would have had.
